Free personal financial statement templates

Blank, lender-ready templates from the team behind LivePFS — generated by the same engine that renders the product’s statements, so what you download is what underwriters actually read. Preview every page before you download. Every file is free, with no email address required.

Which template do I need?

Match the template to what was actually requested — the four documents overlap, and lenders usually want a specific one:

The lender asked for…Use this template
"A personal financial statement" / "a PFS" / their bank’s formThe full personal financial statement — summary, schedules, declarations, certification. Filling it in also pre-answers any bank’s own form.
"A schedule of your properties" / "an SREO"The real estate schedule — one column per property with value, debt, and cash flow.
"A list of your debts" / SBA Form 2202 prepThe debt schedule — one row per obligation with balance, terms, and collateral.
"A net worth statement" / a quick snapshotThe net worth statement — the one-page balance sheet with computed totals.
"SBA Form 413"Not a template — the SBA’s own form, which LivePFS produces from a maintained statement (see Lender formats). The full PFS template is the worksheet behind it.

When in doubt, complete the full personal financial statement — the other three are expansions of its sections, and a finished PFS answers most follow-up requests by transcription. Asked for SBA Form 413 or your bank’s own form? Those aren’t blank templates — see Lender formats for the formats LivePFS produces from a maintained statement.

Questions, answered

Are these templates really free?

Yes — every file downloads directly, no email or account required. They carry LivePFS branding and are free to use and share, including in loan-prep resource lists.

Which template do I need?

If a lender asked for a personal financial statement, use the full PFS template. If they asked for a schedule of your properties, use the real estate schedule. For a list of your obligations — or SBA Form 2202 prep — use the debt schedule. For a quick one-page snapshot, use the net worth statement.

Will lenders accept these?

The templates follow the standard bank-form structure — summary, schedules, declarations, certification — which most lenders accept when complete and signed. Lenders that require their own form still get a faster, cleaner transcription from a completed template.
